A parent and or legal guardian must complete and submit a
registration form to the Forestville Boys & Girls Club (FBGC). No one under the age of 19 is allowed to register
a player. By submitting this registration form the parent or legal guardian understand that FBGC is run exclusively
by volunteers. They also understand that they will be asked to volunteer
as well.
Registration
discounts may be available for households registering three or more members in the same household.
Please see a member of registration for more information. A copy of your child's birth certificate and a small
picture is required for all new members.
New members must complete the FBGC Membership Application, print a copy and bring to registration.
